Why do some survey takers see 3 comment boxes, or all questions at once?

Every once in a while a survey taker will see three (3) comment boxes at one time, and may see all survey questions at once.  This rarely happens, and when it does, it is due to the survey taker using a very old browser, or they have disabled their browser from using Javascript.  Javascript is a very common web technology used across the Internet.  We use Javascript to hide and show survey questions. 

If someone is using a very old browser, or they have disabled Javascript in a newer browser, we are unable to use Javascript to change the survey questions based on their choices.  As a result, the survey displays all the possible questions.  Furthermore, this user's overall Internet experience is significantly negatively affected since Javascript is used on many sites, including many popular shopping sites and social sites.  

The solution is to have the customer enable Javascript in their browser settings, and reload the survey.
